Job Title: Clinical Pharmacy Speacialist

JOB PURPOSE:
The role holder is responsible for performing as pharmacy caregiver by efficiently designing, recommending, activating, monitoring and evaluating patient specific pharmacotherapy plan to ensure optimal outcomes. Maintain liaison relations with medical and nursing staff. Servs as a clinical resource for other pharmacy personnel, medical and nursing staf as well as other health care professionals.

Key Responsibilities:
• Participates in medical rounds to obtain pertinent information required to determine the necessity of drug therapy, and provides input into drug selection and monitoring.
• Determines accurate Medication Related Problem list and records as necessary.
• Obtains pertinent information required to determine the necessity of drug therapy.
• Consults and liaises with Physicians with regards to drugs without indications.
• Selects drug products that are effective, cost-beneficial, and promotes patient compliance.
• Suggests the appropriate therapeutic alternatives for Non-Formulatory drugs.
• Recommends dosage alteration or drug discontinuation when indicated, participates in the writing and evaluation drug of therapy protocols.
• Identifies potentially significant drug-drug/food/laboratory/disease interactions and communicates the clinically relevant interactions to health care practitioners with recommendations of therapeutic alternatives.
• Provides pharmacokinetics consultation for agents requiring such monitoring.
• Provides and evaluates drug therapy orders for appropriateness of dosage, route, interval, schedule, and duration throughout the patient's hospital course.
• Evaluates independently the patient response to drug therapy.
• Assures compliance with the adverse drug reaction reporting program and medication errorreporting programs.
• Records recommendations, intervntions or other appropriate activity in the medical record and Therapeutic Intervention Documentation program.
• Provides educational presentations to the Pharmaceutical Care Services entity.
• Assists medical staff with the selection of drug therapy and patient monitoring.
• Reviews, verify and interprets medication orders for clarity, accuracy, rationality, compatibility, feasibility and suitability for the patient's specific needs.
• Assumes responsibility for the patient-specific pharmacotherapy plan to ensure optimal outcomes.
• Develops and evaluate therapeutic protocols.
• Conducts daily systematic medication reviews for patients in other floors other than the floor covered by the clinical pharmacy specialist. whenever assigned.
• Participates in committees relevant to areas of service.
• Enters and processes medication orders whenever scheduled to by the area accountable.
• Determines workload priorities, verifies medication orders check the final product for accuracy and assures timely delivery of the medications to the patients care area.
• Acts as drug information resource for the patient, students, trainees, interns, colleagues, medical and nursing staf and other healthcare professionals.
• Documents therapeutic recommendations in the patient's medical record and pharmacy J intervention forms
• Provides real-time collaboration with nurses to create cost-effective medication care plans; focusing on medication misadventure prevention.
• rovides educational mentorship to students, interns, trainees, R1, R2 and R3 residents in KSUMC.
• Contributes to positive outcomes and meeting the cultural exceptions of the organization by exhibiting professional behaviour and expertise.
• Monitor patient on high alert medications and provide recommendation and monitoring on patient I care.
• Writes an official delegation of all responsibilities and assignments prior to leave/ vacation.
• Partcipates in the departmental performance development, educational and other clinical activities.
• Participates in self and other educational, training and development as applicable.
• Follows all related general organization's policies and procedures.
• Attend staff and unit meetings as requested by the department.
• Perfrms other job related duties as required by Pharmacy Administration.

Requirements

• Bachelor Degree in Pharmacy/ Doctor of Pharmacy Degree (PharmD) with Diploma in Specialty Pharmacy accredited Saudi Commission for Health Specialties (SCFHS) or American Society of Health-System Pharmacist (ASHP) OR Master Degree in Clinical Pharmacy from an accredited college of pharmacy.
• Certificate from the Board of Pharmacy Specialties (BPS)
• Current Registration from Country of Origin.
• Valid registration from Saudi Commission for Health Specialties as Registered Pharmacist.

Years & Nature of Experience/Age
• Bachelor/ PharmD with Diploma: Three (3) years work experience in pharmacy field.
• Master Degree: Four (4) years work experience in pharmacy field
