Scope of Service: Home Health Care
Monthly Patient Visits: 350-400

Scope of Service:
The Home Health Care Program provides and supervises nursing care of KFSH&RC patients in their own homes, for the promotion of their physical, psychological and social well-being, and provides patients, family and caregiver support.

Up to date, the average patient census of the HHC Program is 115. Approximately 90% of the patients are adult, 10% pediatric. An average of 20% of the patients seen are referred by the Palliative Care Service and the remaining 80% are referred by other services such as Neurophysiology, Internal Medicine, Oncology, and Pediatric Hematology. Those patients who are not considered to require palliative care but have been identified as being at high risk for re-admission to the hospital are accepted into the Program on a short term basis to assist, educate and support them during the transitional stage from hospital to independence in self-care in the home setting.

Special Programs:
To improve patient care and extend this care to a larger patient population, in HHC there are several programs that are functioning/under study aiming to provide service to more patients. Those programs are: administering intravenous antibiotic at home and transfusing paitents with morphine infusion at home aiming to control their pain or other symptom management; currently under study is 'home helth care ventilated patients' program.

Patient Assessment
• Assessment of patients at home depends on the nature of patient's disease, needs, and to what extent the patient/caregiver is independent in providing care. When the first visit is performed, all patients are fully assessed, including a detailed history and physical assessment, and screened for nutritional or functional risks as well as any other identified needs such as educational. Thereafter re-assessment is done as:
• For non-palliative patients are re-assessed with each visit. This includes VS, pain assessment, and assessment of either newly identified patient needs or as part of evaluating previously identified needs.
• For palliative (end-of life) patients re-assessment includes assessment of pain and assessment of any other symptoms that require management. Other assessments will be done based on patients acute needs or according to treatment protocol.
• The frequency of visits differs from one patient to another depending on patient's needs.

REQUIREMENTS

• Western Trained Registered Nurse with current nursing license from country of origin
• Two years recent acute care nursing experience
