Home Care Nurse

JOB DESCRIPTION / ROLE

Employment: Full Time

The Home Care Nurse is responsible for providing high-quality, compassionate medical care to patients in their homes. This role involves assessing patient health, creating care plans, administering medications, and collaborating with other healthcare professionals to ensure optimal patient outcomes. The Home Care Nurse must demonstrate excellent clinical skills, strong communication abilities, and a patient-centered approach to care. They should be able to address all the caregivers’ concerns and make appropriate adjustments to the patients’ in-home management plan.

Requirements:

• Patient Assessment: Perform comprehensive assessments of patients’ physical, emotional, and social needs in their home environment.
• Care Planning: Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and healthcare team members.
• Medication Management: Administer prescribed medications and treatments, monitor for side effects, and educate patients and caregivers about medication management.
• Wound Care: Provide wound care and manage other medical needs, such as catheter care, injections, and IV therapy.
• Health Education: Educate patients and their families about managing illnesses, maintaining a healthy lifestyle, and using medical equipment.
• Documentation: Maintain accurate and timely records of patient care, including assessments, treatments, and progress notes.
• Communication: Maintain clear and open communication with patients, families, and other healthcare professionals to ensure coordinated care.
• Emergency Response: Recognize and respond to emergencies appropriately and provide first aid or other interventions as needed.
• Patient Advocacy: Act as a patient advocate, ensuring that patients’ rights and preferences are respected and that care is delivered in a dignified manner.

Salary:
AED 4,000 to 5,000 per month inclusive of fixed allowances.
