Job description / Role
Responsibilities:
• Undertake Social Care Assessments and reviews to help residents to identify their needs and desired outcomes which places them at the centre of all interventions and giving them choice and control over the delivery of services.
• Protect the rights and promote the interests of Hounslow residents and their carers to ensure they are kept safe, in line with the Council's and Pan London Safeguarding Policy including contributing to planning meetings.

Requirements
• Actively support partnership working with colleagues, partner agencies, voluntary sector etc to ensure a holistic assessment if completed
• Ability to work with communities, partner organizations and voluntary sector to maximize social capital for vulnerable residents and ensure support is culturally appropriate
• To maintain accurate, detailed and full records of interventions and record using IT system. This may include the provision of advice, assistance, evidence and written assessments
