Description
Scope of Service:SOLID ORGAN TRANSPLANT (B2)

Scope of Service:
There are 33 beds in B-2 dedicated to Liver, Renal and Lung Transplantation. The unit provides intense and complex nursing care to surgical adult and renal transplant pediatric patients. Evidence based care, treatments and surgical interventions are applied to provide patients with the best possible outcomes. Nursing in B-2 is challenging, diversified with a focus on primary nursing. Several of the patients' rooms have negative pressure facilities to meet patient's needs. B-2 is complemented by the expertise of highly esteemed physicians and surgeons who work together with the nurses to achieve safe, competent and positive patient outcomes. The unit has designated specialized Health Educators who educate and are an additional resource to the team. The unit performs renal, liver and tissues biopsies in our specialized procedure room. The procedure room houses a small ultrasound machine to facilitate these procedures.

B-2 also provides clinical rotation to nursing students, interns and newly graduated nurses.

Frequent Procedures/Services/Functions:

Management and care of patients, pre and post renal transplantation:
* Cadaveric transplantation
* Living related/non related transplantation
* Negative cross-match and ABO incompatible transplantation
* Immuno-adsorption
* Therapeutic plasma exchange and plasmapheresis (TPE)
* Infusion of IVIG and anti-thymocyte globulin, Ritimab
* High dependency 1:1 nursing care post transplantation for 24 hours only.
* Cc per Cc fluid replacement and management

Management and care of patients with liver diseases:
* Cadaveric/Orthotopic liver transplantation
* Living related/non related transplantation
* Ca of liver
* Hepatocellular Ca
* Cryptogenic liver cirrhosis
* End stage liver diseases
* Viral hepatitis B & C
* Liver resection
* Infusion of IVIG, anti-thymocyte globulin
* Hepatic encephalopathy
* Pre-liver transplant work-up

Management and care of patients undergoing Lung Transplant
* Pre and Post-Operative Care

Hours of operations:
24 hours daily, 7 days a week

CRITERIA FOR ADMISSION TO THE SERVICE
The main criteria for admissions into the unit include all the above services. Patients with ESLD are admitted for further medical or surgical intervention as needed. In some instances, 1:1 and 1:2 Nursing is sometimes provided to post living related/non related transplantation. The patient should not require cardiac monitoring, intubation,inotropes medication or cordis large bore catheter or arterial access lines.

STAFFING PLAN
Number and Mix of Staff

The criteria for staffing is guided by pre-assigned NHPPD (nursing hours per patient day). One nurse in day shift is assigned as Charge Nurse and does not have a patient assignment.

Shift assignments are prepared by the charge nurse and based on the following:
* Be guided by the NHPPD
* Complexity of patients condition and required nursing care
* Dynamics of patients status, including frequency with needs for specific nursing care activity changes
* Type of clinical treatments employed in providing nursing care
* Degree of supervision required by each nursing staff member based on his/her level of competency.

Method of adjusting to staffing variances:
When more staff is required than scheduled, the Nursing Supervisor office is notified. Program Director is informed of any change in acuity and dynamics of the department.

Position Title: Staff Nurse
Reports To: Head Nurse or as assigned

ROLES AND RESPONSIBILITIES:

Summary:
Provides safe patient and family focused quality care utilizing the nursing process. Demonstrates leadership competencies while in charge or acting as shift resource/team leader. Contributes to the successful achievement of the Nursing Affairs strategic goals.

Essential Responsibilities & Duties:
* Provides total patient care as assigned according to the core standards of patient care and established policies and evidence based practices.
* Assumes accountability for assessing, planning, implementing and evaluating care as well as discharge planning for each patient.
* Contributes to decreasing the length of stay/waiting time as per unit specific goal. Works in collaboration with Case Management to achieve goals.
* Documents all relevant information correctly, appropriately and in a timely manner.
* Reports equipment failures, safety and security issues, and/or reduction of supply inventory to Head Nurse or designee.
* Demonstrates collaborative multi-disciplinary partnerships working towards the achievements of constant improvement in quality patient care. Acts as an advocate for patients and families. Identifies ideas that will improve the quality of patient care and issues that will negatively impact on patient care outcomes, using the professional practice framework. Participates through team working and collaborative partnerships in identifying and implementing solutions through evidence based practice or research to demonstrate improvements reflected at unit level.
* Promotes a safe environment and safe patient care by embracing a safety culture and identifying areas of risk to Head Nurse or designee. Complies with policies for personal and patient safety and for the prevention of healthcare associated infections.
* Contributes to the achievement of cost efficiency savings on unit. Maintains an awareness of resource utilization, exercising care in the ordering and use of staff, equipment and materials.
* Contributes to the achievement of clinical quality outcome indicators and patient satisfaction. Leads unit based Performance Improvement activities as assigned. Contributes to special projects/assignments linked to nursing strategic priorities and professional practice model.
* Serves on unit based and divisional councils as assigned/elected. Contributes to the development,implementation and evaluation of unit activities.
* Participates in the development and evaluation of IPPs.
* Assists in promoting and sharing knowledge via mentorship in line with relevant research and evidence based practice.
* Participates in patient education programs.
* Develops and sustains own knowledge, clinical skills and professional awareness and maintains a professional profile. Provides documented evidence of performance and maintenance of skills consistent with position.
* Participates in new staff/nursing student orientation aimed at developing competence and performance. Assists in maintaining a suitable learning environment and acts as a preceptor/mentor in the supervision and teaching of nursing students and new staff to develop their competence.
* Demonstrates transcultural principles in nursing practice.
* Participates in the promotion and maintenance of effective communication and teamwork. Treats colleagues, patients, and visitors with dignity and respect at all times.
* Actively welcomes and provides support for new staff/students to the unit. Contributes to the retention of all staff to meet the unit specific turnover rate goal. Participates in retention activities.
* Upholds the organizational values at all times.
* Follows all Hospital related policies and procedures.
* Performs other related duties as assigned.
