Job description / Role

Employment: Full Time

Role and Responsibilities:

Assessment

Conduct thorough head to toe assessment on each patient at home visit on the shift. For ex:
• Pain assessment
• Nutritional status
• Bed ridden patient for skin integrity
• Fall assessment

• Conducts age specific assessment of patients
• Provides age specific monitoring of patients
• Provides age specific management of patients requiring special care
• Assesses patients/families health education needs and intervenes accordingly
• Completes re-assessment of assigned patients by utilizing nursing process to identify care needs and documents it accurately in the appropriate form

Planning
• Plans and prioritized his/her work to provide best quality care according to patients need

Implementation
• Delivers planned care (Total patient care) to the patient
• Administers medication safety, monitors patients responses to medication where indicated and reports recalled incident promptly
• Positioning – move the patients or body part to provide comfort. Reduce the risk of skin break down, promote skin integrity and to promote healing
• Code Management – nurse should be able to coordinate with the doctors, hospital emergency and ambulance to shift the patient immediately during life threatening situations
• Neurologic Monitoring – collects and analyses patient data to prevent or minimize neurologic complications
• Fluid/Electrolyte management
• Demonstrate safe handling and proper use of equipment
• Participates actively in patient/family education
• Provides safe environment for the patients and him/herself by following infection control policies and applying safety measures for patients
• Monitors and records nutritional status of all assign patients in patients chart and informs dietitian whenever required
• Follows discharge procedure of his/her assigned patients according to hospital protocol. Multidisciplinary Care Conference – plans and evaluates patients care with professionals from other disciplines

Implementation
• Evaluate results of nursing interventions on the patients and modifies nursing care plan
• Ensures all assign patients got quality nursing care by reassessing patient’s health status as per requirements and before his/her shift ends.
• Evaluate the level of patients satisfaction at the end of his/her shift

Documentation
• Provides timely, comprehensive and complete nursing documentation as per nursing division's protocol/standards

Salary:
AED 6,500 to 7,000 per month inclusive of fixed allowances.
Additional benefits: transportation, medical insurance, 2 years visa

Requirements

Candidates should have the following:
• Gender - Female
• License - Letter of Eligibility or HAAD License
• Language: English, basic Arabic (preferred)
• Age: Less than 50 (preferred)
• Location: UAE (preferred)

Qualifications and Education Requirements:
• Certified nurse with HAAD license
• This position requires the incumbent to be experienced in Home care Management. Previous experience managing a health service is essential. Able to demonstrate sound clinical practice.

Physical & Environmental Requirements:
• Ability to provide cover for extended working hours
• Able to shift between different tasks within specific time frame
• Taking and hearing occur continuously in the process communicating with the staff
• Walking and standing are required as part of the job, however the length of these activities will vary from day to day and task to task
• Able to sit on desk for up to 8 hours per day
